You start this by telling him that you have sinned. He asks how. You must let him know that you have severe homicidal tendencies. Telling him you've killed people is peanuts for this guy, you need to raise the stakes if you want something more. He will task you with killing every child in the game. Should be somewhere between 660 and 704. The number is entirely variable and it's not sure what are the limits of this range. This task is not logged in the PipBoy as a quest. Accepting will yield the following message in your event log:

Accepting this offer will spawn a yellow encounter anywhere in the world map with lots of children in it. Once they are all killed, the event log will say:
