Poison damage is done as additional damage by the: Needler Pistol, Poisoned Throwing Knife, Pashtshuur Spike, or scorpion sting. It can be serious and even fatal.
More Information
The character was most likely attacked by one of the poisonous creatures in the wasteland such as the common Radscorpion, or Floaters or Centaurs, or could have been hit by a Needler Pistol. You can be poisoned by simply visiting certain locations, like Toxic Caves. Being stung by Black Claw Radscorpions has killed many adventurers. In any case, the poisoned character will receive an amount of damage over time until the poison has run its course, or has been neutralized by some Antidote.
