Frier's family of six lives in a building in the north corner of Junktown, and he's concerned about the health of his four children. He can barely afford to pay the local doctor, so maybe you can help.
NOTE: First you examine and diagnose one of his daughters, but via dialogue with Frier, not by talking directly to any of the children. Then find out it's his son Trinkered who has the worst problem. There is no apparent way to examine the other two children though, so it's possible this quest is unfinished or partially broken.
Caution: In your dialog with Frier you can easily spend 10k caps for him to pay Doc Larrikin, and that has happened accidentally. There is no XP for that option though, so offer your own Doctor Skills instead. Also, you have to stand specifically close to Frier - if you stand in the wrong spot, even if it is close to him, dialogue will keep ending before it's supposed to. Find the right hex.
